This position is responsible for acting as a liaison between the SSVF grantee and the VA or community medical clinic and works with a population of Veterans throughout the tri-county, with complex needs who require assistance accessing health care services or adhering to health care plans. The Health Navigator acts as an advocate for the client, integrating the Veteran's cultural values into their care plan. The health care navigator assists the Veteran in identifying methods to monitor progress toward meeting health goals and provides ongoing follow-up. A day in the life of our SSVF Healthcare Navigator
  • Develop a trusting and working relationship with the veteran/veteran's household seeking help.
  • Conduct non-clinical assessments of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members, and significant others.
  • Collaborate with the team and the Veteran to identify and address systems challenges for enhanced care coordination as needed.
  • Assist veterans in communicating their preferences in care and personal health-related goals to facilitate shared decision-making of the Veteran's care.
  • Serve as a resource for education and support for Veterans and families and help identify appropriate and credible resources and support tailored to the needs and desires of the Veteran.
  • Participate in the development of the Veteran's care plan with emphasis on community services, outreach, and referrals needed for the Veteran.
  • Provide case management for clients by identifying available resources, strengths, and supports that can support the goals in their individualized health plan, as well as any risk factors and unmet service needs.
  • Identify systemic barriers within the organization, communicate with organizational leadership about these barriers, and works collaboratively to find viable solutions.
  • Regularly review care plan goals with the Veteran, conduct regular non-clinical barrier assessments, and provide resources and referrals needed to support adherence. Evaluate the effectiveness of resources and referrals provided and make appropriate modifications to ensure the provision of high-quality care and interventions.
  • Cultivate positive relationships with relevant funding and monitoring entities, faith-based organizations, social service providers, and other community partners. Participate in appropriate community collaborations. A few things all our team members do.... Teamwork

Position requirements include:
2 years of applicable experience in social work or health services Willingness and ability to be trained in navigating VA services Must have strong communication skills including: being comfortable speaking to groups, provide a welcoming presence to clients, communicate with others non-judgmentally, interact appropriately with the public in stressful situations, communicate in a timely manner, honestly, effectively & transparently in all aspects of the job Ability to maintain a professional demeanor at all times including occasionally de-escalating agitated clients and stressful situations Ability to accurately interpret and execute compliance requirements for various programs Knowledge of trauma informed care principles Microsoft Office suite software; including MS Word, Excel, Power Point, Publisher & Outlook Must have strong data entry skills and ability to work with spreadsheets, data reports, etc. Valid Oregon Driver's license with driving record that is acceptable under COVO's insurance policies Willing and able to drive throughout Deschutes, Crook and Jefferson counties including winter travel conditions Being a part of the COVO team means : Our community of care extends to homeless persons, residents, vendors, consumers, staff, outside agencies, and all people who enter our dwelling places.
