Patient Navigator- Meadows Mammo (Full-Time)

Responsibilities :

Assess patients' social, emotional, and environmental needs and connect them with appropriate resources and services. Serve as a liaison between patients, families, care teams, and community organizations to reduce barriers and support care coordination. Guide patients through the continuum of care, from screening and diagnosis to treatment, survivorship, and end-of-life care. Provide individualized navigation services to address financial, social, and access-to-care challenges. Coordinate support services including transportation, insurance, financial assistance, and community resources. Facilitate clear communication by arranging interpretation services and supporting patient understanding during visits and follow-up. Assist with scheduling, registration, and completion of forms to ensure a smooth patient experience. Support cancer screening programs by coordinating appointments, obtaining orders, and promoting early detection initiatives. Conduct patient outreach, education, and follow-up to ensure engagement and continuity of care. Document patient interactions and program data while supporting reporting requirements and quality initiatives.

Schedule:

This position will work Monday-Friday, 8AM-4PM. May require travel to off-site locations for community events. Events may be other times or days than typical schedule. Bachelor's degree in a health services related field required. Community patient navigation experience with bilingual interpretation services experience preferred. Experience in Oncology preferred.

Licensure, Certifications, and Clearances:

CPR required based on American Heart Association standards that include both a didactic and skills demonstration component within 30 days of hire.
