Schedule:

9am-6pm PST What you'll do Make 20-70 outbound calls/day; land ~10 patient check-ins Schedule visits, labs, imaging, and outside referrals Coordinate care between providers and patients (clear ownership) Gather medical history and document in EHR Follow up after visits with next steps Escalate issues to clinical team (fast feedback) Keep patients engaged and supported on calls Must-haves Comfortable making high-volume outbound calls daily Basic medical terminology knowledge Experience speaking with patients/customers by phone Clear, professional communication skills Able to adapt in a fast-changing environment Nice to have Care coordination or patient navigator experience Spanish language skills Experience with cognitive/elder care populations Perks & pay
