This is a high-impact outreach role. You will be responsible for identifying eligible Medicare patients, educating them about our monthly care coordination services, and successfully enrolling them into the program. We are looking for someone confident on the phone, organized, resilient, and motivated by measurable results. Job Summary The CCM Enroller plays a vital role in connecting patients with structured chronic care support. This position requires daily outbound calls, clear communication, accurate documentation, and consistent productivity. Your ability to confidently explain the program and guide patients through enrollment will directly impact patient care and company growth. Key Responsibilities Review patient medical records to confirm CCM eligibility (two or more chronic conditions) Conduct high-volume outbound calls daily Clearly explain CCM services, benefits, and expectations Obtain and document verbal consent in compliance with HIPAA guidelines Schedule initial care plan appointments Assign enrolled patients to Care Coordinators Maintain accurate documentation in ThoroughCare Track daily call volume, conversations, and enrollment metrics Address patient questions or objections professionally and confidently Collaborate with care coordinators and providers to ensure smooth enrollment transitions Support outreach efforts as ElevateCare Connect expands to additional physician offices Qualifications Prior experience in telemarketing, sales, medical office outreach, insurance sales, or call-center roles preferred Strong communication and persuasion skills Comfortable discussing healthcare services and addressing objections Organized, detail-oriented, and able to multitask efficiently Knowledge of HIPAA and patient privacy standards Self-motivated and able to work independently in a fast-paced environment Bilingual candidates encouraged to apply Why Join ElevateCare Connect We are building a structured CCM department focused on proactive, coordinated patient care.
