Job Description

Overview Cannon Pharmacy is seeking a Certified Medical Assistant (CMA) to coordinate and support our Remote Patient Monitoring (RPM) program. This innovative healthcare initiative enables the remote tracking of patients' health through cellular-connected blood pressure and blood glucose monitors. The CMA will lead engagement with eligible patients, manage patient enrollment, coordinate Telemedicine visits, and oversee ongoing patient monitoring tasks playing a vital role in improving patient outcomes and supporting sustainable revenue for the pharmacy. This position is initially part-time, with strong potential to transition into a full-time role following a work period performance review. Additionally, there is future opportunity for this role to evolve into a hybrid remote/in-person position, offering increased flexibility as the program grows.
Key Responsibilities:
Patient Enrollment & Onboarding:
Serve as the primary coordinator for new patient RPM enrollments. Educate patients on the program, its health benefits, and proper device use. Facilitate a live Telemedicine visit between the patient and an independent provider during enrollment appointments. Collaborate with the pharmacist for a brief introduction at the start of each new patient visit. Confirm patient eligibility through insurance verification and documentation.
Ongoing Monitoring & Care Management:
Review patient-submitted vital signs daily via the RPM portal. Identify abnormal readings and escalate clinical concerns to supervising pharmacists or providers. Perform regular care management duties, including patient communication, record updates, and device troubleshooting. Ensure monthly monitoring compliance and follow-up scheduling.
Program Workflow Implementation:
Utilize standardized workflows, updated task lists, and training protocols for optimal program execution. Maintain accurate records and support quality assurance efforts. Assist with data collection related to patient outcomes and program performance.
Program Impact:
Contribute to meaningful patient outcomes, including a 10 mmHg average reduction in systolic blood pressure among hypertensive patients enrolled for 90+ days. Help patients achieve a 20% reduction in cardiovascular event risk through consistent monitoring and intervention. Be part of a team delivering high-value care while supporting a profitable, sustainable service line for the pharmacy.
Required Qualifications:
Certified Medical Assistant (CMA) with active certification. Experience in outpatient care, chronic disease management, or care coordination. Excellent communication and patient engagement skills. Comfortable using Telemedicine platforms and clinical monitoring software. Organized and able to work independently while managing multiple priorities.
Preferred Qualifications:
Experience with Remote Patient Monitoring or population health management. Familiarity with pharmacy workflows or community health settings. Bilingual skills are a plus.
Pay:
$18.00 - $20.00 per hour
