Job Description

As a Clinical Partnerships RN, you will represent our organization in the community, building trusted relationships with physicians, practices, and care teams. You will communicate the value of our services, support appropriate patient referrals, and serve as a key bridge between external partners and our internal clinical team. This is a dynamic, outward-facing role that combines clinical expertise with relationship-building, strategic thinking, and community engagement. Key Responsibilities Develop and maintain relationships with physicians, specialists, and healthcare organizations Educate providers on our services, care model, and appropriate patient fit Serve as a clinical liaison between internal team and external partners Identify opportunities to support providers and improve care coordination for shared patients Facilitate introductions and onboarding of new referral partners Represent the organization at meetings, events, and provider offices Collaborate closely with the internal clinical team to ensure alignment and continuity of care Track outreach efforts and partnership activity Qualifications Active California RN license Preferred 10+ years of nursing experience , including acute care Strong clinical knowledge and ability to communicate confidently with providers Excellent interpersonal, communication, and presentation skills Experience in care coordination, case management, or community-based roles preferred Self-directed, organized, and comfortable working independently Professional presence with the ability to build trust quickly Valid driver's license and willingness to travel locally What We Offer Flexible, part-time schedule (15-20 hours/week) Autonomy and variety in your workday Opportunity to influence care beyond individual patients Collaborative, mission-driven team Meaningful role in improving access, coordination, and quality of care Why This Role This is an opportunity for a seasoned nurse to step into a strategic, relationship-driven role —helping shape how care is delivered across the community.
