  • We are seeking a compassionate and detail-oriented Integrated Case Manager to join a patient-centered care team.
This role is responsible for assessing, planning, coordinating, and evaluating care for patients with complex healthcare needs across the care continuum. The goal is to ensure safe transitions, improved health outcomes, and effective use of healthcare resources. This position works across ambulatory, inpatient, and health plan settings and supports patients who require complex care management following critical health events or diagnoses. Key Responsibilities
  • Conduct comprehensive assessments of patients' biomedical, psychological, social, and functional needs Develop and implement individualized, patient-centered care plans Advocate for patients and families through education, communication, and resource coordination Support patients in overcoming barriers using motivational interviewing and self-management strategies Provide counseling related to treatment decisions and advance care planning, including end-of-life discussions Coordinate transitions of care to ensure safety and continuity across settings Collaborate with interdisciplinary care teams to manage complex medical and social needs Facilitate care conferences and interdisciplinary communication Monitor and evaluate patient progress and adjust care plans as needed Support process improvement initiatives through data collection and analysis Assist with high-risk cases, including situations involving abuse or neglect Connect patients with appropriate community and health plan resources Perform other duties as assigned Required Qualifications Excellent verbal and written communication skills Strong interpersonal skills with the ability to work with patients, families, and interdisciplinary teams Strong critical thinking, problem-solving, and decision-making skills Proficiency with computer systems and documentation tools Experience in discharge planning, home health, community health, or managed care preferred Knowledge of case management principles, clinical guidelines, and healthcare regulations (Medicare/Medicaid preferred) Understanding of social determinants of health and their impact on patient outcomes Strong organizational skills with the ability to manage multiple complex cases Compassionate, patient-centered approach with strong advocacy skills Education & Licensure•Nursing degree (RN) OR Master's degree in Social Work (MSW) required Active, unrestricted license required: Registered Nurse (RN)•State of Michigan OR Licensed Master Social Worker (LMSW)•State of Michigan OpTech is an equal opportunity employer and is committed to creating a diverse environment.
