Summary:
The Post-Crisis Engagement (PCE) Navigator provides short-term, community-based, in-person support to individuals who repeatedly access crisis services but remain disconnected from ongoing behavioral health, social, and community supports. The Navigator supports participants in post-crisis stabilization, removing barriers to care, and connecting to ongoing resources.
Post Crisis Engagement Navigator Job Responsibilities:
Connect individuals and families to behavioral health, social, and community supports Engage participants in client-driven, non-clinical navigation and service linkage Conduct timely post-crisis outreach to promote stabilization and continuity of care Identify and address barriers to participation, reducing missed appointments, repeated emergency services, hospitalizations, and law enforcement involvement Maintain an assigned caseload (TBD), including scheduling, outreach, follow-up, and documentation Follow up with participants after service conclusion to assess ongoing connection to supports Participate in community forums and crisis system activities as part of the local crisis continuum Complete accurate, timely, and thorough documentation in accordance with agency and program standards Maintain confidentiality and adhere to ethical and legal requirements
Post Crisis Engagement Navigator Minimum Qualifications:
Associate's degree or NY Certified Peer Specialist (NYCPS/NYCPS-P) or NY Certified Recovery Peer Advocate (CRPA/CRPA-P) Three years of professional experience in social, behavioral, or human services Two years of successful independent, community-based work without direct supervision Access to a reliable vehicle with insurance and a valid NYS driver's license
Work Environment:
This position operates in a hybrid work model and is community-facing involving regular travel and in-person engagement in a variety of environments, balanced with work at a Liberty office. Pace and Atmosphere Dynamic, engagement-driven environment requiring flexibility, persistence, and strong interpersonal skills. Navigators work in a highly collaborative environment that emphasizes shared responsibility. Situations may involve individuals experiencing emotional distress, instability, or ambivalence toward services and may require coordination with multiple system partners. Work schedules may include evenings or other non-standard hours to ensure timely post-crisis outreach and follow-up in accordance with program expectations. Worksite Conditions Navigators are required to travel to locations not controlled or maintained by the agency, including private homes, shelters, public spaces, or outdoor environment. Locations may include stairs, uneven terrain, limited accessibility, or other physical challenges. The role may involve standing, walking or other physical activity for extended periods.

Able to deliver stigma-free, culturally responsive services Able to set expectations and maintain personal/professional boundaries Uses adaptable engagement strategies for individual participant needs Effectively collaborates with treatment providers and system partners in a cooperative, collegial manner.
