Monday•Friday, 8:00 AM•4:30 PM (7.5 hours/day) | Hybrid This role serves Wilson and Trousdale Counties, with some travel across Tennessee.
Job Summary:
We are seeking a Community Dementia Navigator to support individuals living with Alzheimer's disease and related dementias (ADRD) along with their caregivers. This position focuses on connecting families with community resources, providing education and memory screenings, promoting dementia-friendly practices, and improving access to healthcare and support services. The role is based in Wilson County and will support Wilson and Trousdale Counties, with occasional travel across Tennessee.
Key Responsibilities:
Build partnerships with community organizations and healthcare providers Connect individuals with dementia and caregivers to local resources and support services Conduct educational presentations, outreach programs, and memory screenings Promote early detection and awareness of Alzheimer's disease and related dementias Track client outcomes, referrals, barriers to care, and program effectiveness Maintain HIPAA-compliant records and confidential client information Support dementia-friendly initiatives and caregiver education programs Participate in ongoing training and stay updated on evidence-based dementia care practices
Required Qualifications:
Bachelor's degree in Public Health, Nursing, Social Work, or related field with 2+ years of related professional experience OR Licensed Practical Nurse (LPN) with 5+ years of experience in healthcare, home health, or community health settings. #
