Strong orientation program, generous tuition allowance and career development What you will do: The Licensed Master Social Worker will provide advanced social work services, care planning, and community resource coordination/navigation to caregivers and families in the community caring for individuals with ADRD. Work involves the use of sophisticated interviewing, assessment, and psychotherapeutic interventions aimed at positively affecting the physical, cognitive, and emotional functioning of our clients. The Care Navigator will assist in identifying appropriate community resources and programs to support our clients and their families to maintain their independence in the community for as long as possible.
Responsibilities:
Conduct Caregiver Assessments and provide Care Navigation services to Caregivers of those with Alzheimer's Disease and other Related Dementias in the Capital Region. Maintain relationships with community partners in the capital region and represent EAS at outreach and sponsored events. Serve on at least one of the departments committees for caregiver education, caregiver wellness, or joint enrichment to provide specialized events for caregivers and the person they care for. Lead our Ambiguous Loss Support Group for Caregivers. Collaborate with EAS team to ensure person-centered services are provided. Demonstrate high-level creative problem solving, organization and advanced communication skills. Demonstrate the ability to work independently and as part of a team. Actively contribute to yearly grant deliverables and take responsibility for reporting events attended/ lead.
Requirements:
Master's Degree in Social Work is required. NYS LMSW or Limited LMSW Permit eligible is required. Minimum of 1 year of experience in the field of Alzheimer's/Dementia and demonstrated knowledge of the disease and challenges for caregivers is preferred.
Pay Range:
$30.00

08 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
