Job Description
Healthcare Navigation Specialist Position Summary/Primary Objective :
The Care Management Specialist/Healthcare Navigation Specialist serves as a vital healthcare navigation specialist supporting veterans in our residential and non-residential programs. Working under the supervision of the Clinical Coordinator, this position focuses on medication coordination, healthcare coordination, and support services. The Care Management Specialist ensures continuity of medical care while serving as a bridge between veterans, healthcare providers, and clinical services. The expectation is that this position will be in-person, with the possibility of occasional remote work, as necessary.Note:

Clinical Coordinator /Clinical Director Clinical Responsibilities:
The Care Management Specialist will collaborate closely with the Clinical Coordinator to provide comprehensive medical case management services to veterans. This position requires expertise in medication coordination, healthcare systems navigation, and understanding of veteran-specific medical needs including physical, mental, and emotional health issues.Job Duties:
Medication Coordination:
Coordinate all medication fills and refills for program participants, monitor- medication compliance and adherence, maintain accurate medication records, communicate with pharmacies and providers, and provide basic medication education to participants.
Healthcare Navigation:
Guide veterans through healthcare systems, schedule and coordinate- appointments, facilitate communication between veterans and providers, assist with insurance processes, and support access to preventive care.
Case Management Support:
Assist the Clinical Coordinator with medical aspects of case management, document observations, provide health education, and support application of medical treatment plans.Care Coordination:
Collaborate on treatment planning and case reviews, participate in- multidisciplinary team meetings, coordinate with community healthcare resources, assist in discharge planning, and maintain communication with families when appropriate.
Documentation:
Maintain accurate and up-to-date records of all clinical activities, assessments, interventions, and outcomes, adhering to confidentiality and privacy regulations.- Other duties as assigned by Clinical Coordinator, Clinical Director, and/or Executive Director.
Qualifications:
Knowledge/Skills/Abilities (Professional Licensure/Certification Not Required, but Preferred):- Bachelor's degree in human services, psychology, social work, or a related field preferred.
- Associate degree in a relevant field with minimum of 2 years' experience in human services accepted. Minimum of 2 years of experience in medical case management, healthcare navigation, or a related role,
- preferably within the veteran healthcare system or human services.
- Understanding of medication coordination processes, healthcare systems, insurance processes, and veteran healthcare needs and challenges. Knowledge of military culture, PTSD, TBI, and other common veteran health issues preferred.
Communication Skills:
Excellent verbal and written communication skills to effectively collaborate with veterans, their families, healthcare providers, and community resources.Organizational Skills:
Strong organizational and time management skills to prioritize tasks, manage multiple cases, and ensure timely coordination of medication coordination and healthcare navigation.Computer Skills:
Proficiency in using electronic medical records (EMR) systems, medication coordination systems, and other relevant software applications.Compassion and Empathy:
Demonstrated ability to provide compassionate and empathetic care to veterans, respecting their unique backgrounds, experiences, and healthcare preferences.Problem-Solving:
Effective problem-solving and critical-thinking abilities to assess complex healthcare situations and develop appropriate solutions for healthcare navigation.Job Type:
