Key Responsibilities:
Care Gap Management:
Identify patients with outstanding care needs and facilitate gap closures through appointment scheduling and follow-ups.
Patient Outreach & Engagement:
Conduct outreach via phone calls, mail, and text messages to encourage patients to attend scheduled appointments.
Data Verification & Reporting:
Review patient lists , verify insurance attribution , and ensure accurate documentation in tracking systems.
Collaboration & Coordination:
Work closely with providers, nurses, and MCO representatives to support HEDIS measures and ER utilization initiatives.
Administrative Support:
Maintain accurate records , assist with scheduling , and support disease management programs .

Required Qualifications:
High school diploma or specialized training/certification (LPN or Credentialed Medical Assistant preferred). 1-2 years in a healthcare-related role with knowledge of medical terminology. Strong communication, attention to detail, and ability to work independently with minimal supervision. Intermediate experience with Microsoft Office (Excel, Word, Outlook).
