Job Description
Header Job Summary Join our dedicated healthcare team as a Patient Navigator, where you will play a vital role in guiding patients through complex medical journeys with enthusiasm and compassion. In this dynamic position, you will coordinate care, facilitate communication between patients and providers, and ensure seamless access to necessary services. Your proactive approach will help improve patient outcomes, enhance satisfaction, and support the overall efficiency of our healthcare delivery system. Responsibilities Serve as the primary point of contact for patients, providing personalized assistance throughout their care continuum Assist with discharge planning, intake procedures, and scheduling to streamline patient flow and reduce wait times Coordinate with multidisciplinary teams including social workers, nurses, physicians, and case managers to ensure comprehensive patient support Educate patients on their treatment plans, medication management, and available community resources Maintain accurate documentation using Electronic Health Record (EHR) systems while adhering to HIPAA regulations to protect patient privacy Conduct utilization review and management activities to optimize hospital resources and ensure appropriate care levels Support patients in navigating hospital services such as
ICU, PICU
(Pediatric Intensive Care Unit), Level I and II trauma centers, and hospice care programs Skills Critical care experience with familiarity in ICU, emergency medicine, or acute care settings Strong hospital experience with knowledge of medical terminology, CPT coding, ICD-9/10 coding systems, and discharge procedures Experience working within EMR/EHR systems and understanding of HIPAA compliance standards Excellent communication skills to effectively educate and advocate for patients from diverse backgrounds Ability to perform case management tasks including intake assessments and utilization review processes Knowledge of social work principles and behavioral health support strategies Familiarity with managed care protocols, utilization management, and discharge planning processes Experience in pediatrics or Level II trauma centers is a plus; OR experience is beneficial for understanding surgical workflows This comprehensive role is designed for motivated professionals eager to deliver exceptional patient-centered care.
