Job Description

The Substance Abuse Navigator is responsible for meeting patients in the hospital when they present due to an overdose, are experiencing active withdrawal, or present with primary substance disorder, to connect them with treatment and recovery services. This is to effect overall care coordination with the emphasis on psychosocial assessment and intervention, complex discharge planning, knowledge of community resources, appropriate documentation in the medical record and networking and collaborating with other hospital disciplines and community agencies. The Substance Abuse Navigator assists the patient, family/guardian, and all members of the healthcare team in the transition of care/complex discharge planning process. Substance Abuse Navigator is accountable for assessments and intervening with patients and family, by applying critical thinking skills to monitor psychosocial status and issues that impact the clinical progression and transition/discharge planning for the patients, coping and decision making. Demonstrates positive communication and interpersonal skills with patients, families and coworkers and participate in patient and family conferences as needed. The Substance Abuse Navigator may be called on to assist with behavioral emergencies beginning in the Emergency Department and continuing through the continuum of care. In addition to performing the essential functions listed below, may also be assigned other duties as required.
Education:
High School Diploma or GED required. F Endorsement driver's license required within 3 months of hire. BLS within 3 months of hire. Certified Peer Recovery Specialist (CPRS) required and must maintain certification in accordance with state standards or be eligible to immediately train to receive their CPRS within 2 months and must be completed within 1 year of hire.
Experience:
1-3 years experience required. Daily computer competency required.
