Job Overview Join our dedicated team as a Maternal Care Navigator, where you will play a vital role in coordinating comprehensive maternal health services through the Delta Maternal Care Coordination Grant. Duties Serve as a primary point of contact for pregnant women, new mothers, and their families, providing education and support throughout pregnancy, delivery, and postpartum periods Assess patient needs through comprehensive intake interviews, including review of medical history, vital signs, and social determinants of health Coordinate access to essential services such as prenatal care, postpartum support, behavioral health resources, and social services Collaborate with healthcare providers across hospital medicine, outpatient clinics, and community organizations to facilitate seamless care transitions and discharge planning Conduct case management activities including diagnostic evaluation, utilization management, and post-acute care planning to optimize health outcomes Maintain detailed medical records in compliance with HIPAA regulations while ensuring accurate documentation of patient interactions and care plans Educate patients on physiology, medical terminology, and health management strategies to empower informed decision-making Provide patient-centered care coordination across the prenatal and postpartum continuum. Conduct screenings and assessments related to social determinants of health (SDOH) (e.g., transportation, housing, food insecurity, interpersonal safety). Maintain accurate, timely, and compliant documentation in electronic systems. Develop and implement individualized care plans to address identified medical and social needs. Facilitate referrals and warm hand-offs to clinical providers and community-based organizations. Track and follow up on referrals to ensure closed-loop referral processes . Support patient engagement and retention in care, including appointment scheduling and follow-up. Collaborate with care teams to support quality improvement (QI) initiatives aligned with HRSA measures. Assist in the collection, tracking, and reporting of program data, including Uniform Data System (UDS) and grant-specific metrics. Participate in outreach and education efforts to increase access to maternal health services in rural communities.
Qualifications:
Experience working in a Federally Qualified Health Center (FQHC) or similar setting Experience with HRSA reporting, care coordination models, or grant-funded programs Prior experience in maternal health, reproductive health, women's health, or pediatric care coordination/case management. Demonstrated working knowledge of health and social service resources specific to West Tennessee's rural counties (e.g., local health departments, community clinics, domestic violence shelters, and food banks), particularly Northwestern Tennessee. Training or certification as a Community Health Worker (CHW), Doula, or similar patient navigation role. EDUCATION/TRAINING
(PREFERRED BUT NOT REQUIRED
): Associate's degree required, preferably in health, social service, or related field. Bachelor's degree in health, social service, or related field preferred. LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of employees and/or the management of organization. MATHEMATICAL SKILLS
Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
