Days Fulltime Responsibilities:
The Care Navigator will identify barriers to care, assist with access issues, develop relationships with health care providers, and ensure effective communications with providers, patients, and caregivers to coordinate services within the organization, at outside facilities, and within the community. The Navigator will track admissions/readmissions, and how patients are helped, and the results of the help given. The Care Navigator will also track and help with Bundle Payments and comprehensive Joint replacement. The Navigator will partner with stake holders to raise awareness about hospital/support programs and services, community resources and needs articulated by patients and families. The Care Navigator is a creative problem solver and relationship builder, responsible for ongoing evaluation and quality improvement of the program.
Qualifications:
EDUCATION, EXPERIENCE, TRAINING 1.
Bachelor's Degree, preferred. 2. Minimum of two years of experience working in a medical setting and interacting with medical teams and patients. Post-acute experience preferred. 3. Knowledge of community resources and community outreach experience. 4. Thorough knowledge of health care systems and medical terminology. 5. Working knowledge of HIPAA regulations. 6. Strong communication, interpersonal, public speaking, and organizational skills; bilingual preferred. 7. Computer proficiency—Microsoft Office suite. Familiarity with databases preferred. 8. Strong problem solving, relationship building and advocacy skills. 9. Ability to prioritize workload, handle multiple tasks, and work both independently and with teams.
