At VitalCaring, our Care Transition Navigators (CTNs) are the critical link between hospital and home - ensuring patients transition safely, avoid readmissions, and feel supported every step of the way. What You'll Do Complete bedside assessments within 24 hours of referral Identify high-risk patients and social barriers to recovery Lead discharge planning conversations with patients & families Partner with case managers, physicians, and hospital teams Coordinate seamless, timely home health admissions Drive 48-hour follow-ups and ensure PCP visits within 3 days Educate patients on care plans, resources, and virtual options Strengthen referral relationships through proactive communication Help reduce avoidable hospital readmissions What You Bring Active RN, LVN/LPN, or PT license 2+ years clinical experience (home health preferred) Strong clinical judgment + relationship-building skills Comfort working in hospital settings (onsite majority of time) Reliable transportation + flexibility Why VitalCaring We don't just deliver care—we transform lives.
