POSITION SUMMARY/RESPONSIBILITIES
In addition to functioning as a patient navigator, the team leader provides training and guidance to the patient navigators in the Community Initiatives and Population Health Department. Works with management to clearly define objectives and priorities, review and assess fidelity, and to coordinate the day to day operations of the team.
EDUCATION/EXPERIENCE
Bachelor's degree in social sciences, health education or related field of study is required. Four years of experience in the area of health education, public health, social work and/or case management may be considered as a substitute in lieu of the Bachelor's degree. English/Spanish fluency is preferred. Leadership experience preferred.
