Job Description

•THIS
IS A TEMPORARY POSITION EXPECTED TO END ON SEPTEMBER
30th, 2026. The HIV Navigator will refer HIV-positive patients to prevention and essential support services. Will be responsible for increasing patient access to care. Position will actively provide testing, education, and resources on HIV, HCV, STD and TB to at-risk individuals within the community. The goal of this position is to assist patients in recognizing behaviors that are placing them at risk and identifying solutions to reduce risks. Patients receiving HIV testing must be provided risk reduction and post-test counseling with referral to ancillary services provided if needed. Position assists individuals who test positive for HIV in accessing services. Position requires familiarity with community resources in order to conduct referrals to services including; substance abuse treatment, STD/TB Clinics, and other services. Provide
HIV/AIDS/HCV
education in both public and clinical settings. Assists senior staff in developing new testing sites and collaborations with other entities. •
THIS IS A TEMPORARY POSITION EXPECTED TO END ON SEPTEMBER 30TH, 2026. ESSENTIAL RESPONSIBILITIES
Visit at-risk areas within the community to test and educate patients Test patients in public and clinical settings Educate patients on resources and services Support and promote collaboration between HIV, STD, Viral Hepatitis, and/or TB programs through the support and provision of integrated screening activities delivered in conjunction with HIV testing Collaborate with key staff of the participating facilities to plan, develop, and implement the integrated screening activities for STDs, viral hepatitis, and/or TB Ensure that patients receive their test results, specially those who test positive Must be able to communicate with patients and public on sensitive issues regarding substance abuse, sexuality and relationships with professional, appropriate, and effective empathy and caring demeanor Ensure that clients who test positive are linked to appropriate medical care and receive timely and appropriate evaluation and treatment Maintain privacy of all patient, employee and volunteer information and access such information only on a need to know basis for business purposes Distribute contraceptives, brochures, and other resources in public places and events
QUALIFICATIONS
High School Diploma or GED required Proficient technology skills required: MS Office applications (Excel, Visio, Word, PowerPoint), Outlook, Microsoft Teams, EHR systems Proficient understanding of HIPAA and patient confidentiality required Proficient verbal and written communication skills required Experience in the human services or medical field preferred
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
Must be willing to work a schedule that includes weekends and evenings Must be willing to access clients in locations where at-risk individuals congregate Must be willing to learn and practice phlebotomy (venous blood draws) Must have reliable transportation
