Records management Health information management Medical terminology Full Job Description Job Summary We are seeking a dedicated and compassionate Clinical Patient Liaison to serve as a vital bridge between patients, families, and healthcare providers within our hospital setting. This energetic role involves coordinating patient care, facilitating communication, and ensuring seamless navigation through complex medical processes. The ideal candidate will possess a strong understanding of hospital operations, medical terminology, and patient-centered care principles. Your proactive approach will help enhance patient satisfaction, streamline discharge planning, and support clinical teams in delivering exceptional healthcare experiences. This paid position offers an exciting opportunity to make a meaningful impact in a dynamic healthcare environment. Responsibilities Act as the primary point of contact for patients and their families, providing clear information about treatment plans, hospital procedures, and available resources Coordinate discharge planning efforts by collaborating with case management, social work, nursing, and clinical teams to ensure timely and safe patient discharges Assist with patient intake processes, including documentation review and verification of insurance coverage using EMR (Electronic Medical Records) and EHR (Electronic Health Record) systems Facilitate communication between multidisciplinary teams to support utilization review and utilization management activities in compliance with HIPAA regulations Support hospital departments such as
ICU, PICU
(Pediatric Intensive Care Unit), trauma centers (Level I & II), emergency medicine, and hospice care by understanding their unique workflows and requirements Maintain detailed records of patient interactions, discharge summaries, and case management notes while ensuring accuracy in
ICD-10/ICD-9
coding and CPT coding where applicable Assist with patient education on medical terminology, post-discharge instructions, and follow-up care plans to promote optimal recovery outcomes Qualifications Proven experience in critical care settings such as ICU or emergency medicine; hospital experience preferred Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and utilization review processes Background in nursing, social work, behavioral health or related healthcare fields highly desirable Familiarity with EMR/EHR systems and HIPAA compliance standards for safeguarding patient information Excellent communication skills with the ability to handle sensitive situations empathetically and professionally Ability to coordinate multiple tasks efficiently within fast-paced hospital environments including Level I trauma centers or pediatric units Experience with discharge planning, case management, managed care, or utilization management is a plus Join us in delivering compassionate care while supporting our clinical teams!
