Job Description

Overview We are seeking a dedicated and knowledgeable Staff Pharmacist to join our healthcare team. The ideal candidate will possess a strong background in clinical pharmacy, with experience in acute care settings, long-term care (LTC) pharmacy, and a commitment to providing exceptional patient service. As a Staff Pharmacist, you will play a crucial role in patient assessment, medication management, and ensuring the safe and effective use of medications. Responsibilities Provide comprehensive pharmaceutical care to patients, including medication therapy management and patient counseling. Conduct patient assessments to evaluate medication needs and outcomes. Collaborate with healthcare professionals to develop and implement individualized treatment plans. Compounding medications as required, following aseptic techniques and safety protocols. Administer immunizations and manage medication administration processes. Maintain accurate inventory management practices to ensure the availability of medications. Utilize pharmacy software and electronic health record (EHR) systems for documentation and patient care activities. Monitor patient responses to medications, particularly in critical care and pediatrics settings. Ensure compliance with HIPAA regulations and maintain confidentiality of patient information. Participate in pain management strategies and provide education on medication use for acute care patients. Requirements Doctor of Pharmacy (Pharm.

D.) degree from an accredited institution is required. Completion of a pharmacy residency or equivalent clinical pharmacy experience is preferred. Experience in LTC pharmacy or hospital settings is highly desirable. Strong knowledge of medical terminology, physiology, anatomy, and pharmacology. Proficiency in IV insertion and IV infusion techniques is an asset. Familiarity with EMR systems for efficient patient documentation is essential. Excellent communication skills with a focus on patient service and care coordination. Ability to work collaboratively within a multidisciplinary team environment.
