Indian School Rd., Outpatient Pharmacy, Building 8, Room A1215, Phoenix, AZ 85012 Inpatient Pharmacy, Building 1, Room 3108, Phoenix, AZ 85012 32nd Street Outpatient Pharmacy, Phoenix, AZ 85012 The first two locations are on the main campus. The 32nd Street Outpatient Pharmacy is located near the main campus. Position Overview We are seeking experienced Licensed Pharmacists to provide inpatient and outpatient pharmacy services at the Phoenix VA Health Care System. Pharmacists will deliver a full range of pharmacy services to VA beneficiaries in accordance with VA standards, state and federal regulations, The Joint Commission standards, and facility policies. Key Responsibilities Provide inpatient and outpatient pharmacy services in a VA healthcare setting. Dispense medications prescribed by physicians and other authorized healthcare providers. Review medication orders for appropriateness, accuracy, therapeutic duplication, allergies, drug interactions, drug-food interactions, incompatibilities, side effects, dosing intervals, and dosage strength. Required Qualifications Must be a U.S. Citizen or Green Card holder . Must be a graduate of an ACPE-accredited College or School of Pharmacy . Must have a B.S. Pharmacy and/or Doctor of Pharmacy (Pharm.

D.) degree . Must have a full, current, and unrestricted pharmacist license in any U.S. State, Territory, Commonwealth, or the District of Columbia. Must be licensed for controlled substances and qualified to practice in any state. Preferred Qualifications At least 24 months of relevant pharmacist experience within the most recent 36 months preferred. Prior inpatient and/or outpatient pharmacy experience preferred. Prior VA, federal healthcare, hospital, or high-volume outpatient pharmacy experience preferred. Shift Expectations The facility operates 24 hours per day. Pharmacists may be assigned varying schedules based on staffing needs.
Possible shifts may include:
