Automotive Description At NorthBay Health, the Pharmacy Resident is hired into a 12-month postgraduate training program. Reporting to the Pharmacy Clinical Coordinator, the Resident initially operates as a pharmacy intern under the direct supervision of a licensed pharmacist; upon obtaining licensure from the California State Board of Pharmacy, the Resident will transition into the full clinical and operational responsibilities of a Clinical Pharmacist. The successful resident will meet the requirements established by the American Society of Health-System Pharmacists (ASHP), the accrediting body for pharmacy residency programs in the United States.PRIMARY JOB DUTIES
Perform clinical patient care (e.g. acquire patient medication history, manage medication therapy including ordering medications and laboratory tests under protocol, perform pharmacokinetic monitoring, optimize drug therapy for patients in various acute and ambulatory care areas, evaluate medication errors and adverse drug events, and document work in the electronic health record.) Participate in interdisciplinary teams (e.g. participate in clinical rounds and collaborate with physicians, nurses, and other healthcare professionals.) Execute staffing & operations responsibilities (e.g. prepare/check unit dose medications, compound/check IV medications, manage narcotic security, review/approve drug orders, and staff shifts, including some weekends.) Respond to codes (e.g. manage drug preparation and administration during Code Blue and Code Stroke events.) Plan, design and complete research and other projects (e.g. a major residency project, medication use evaluation (MUE) or quality improvement projects.) Provide education (e.g. medication counseling to patients, in-service education for staff, precepting activities for pharmacy students.) Provide leadership (e.g. participate in Pharmacy & Therapeutics Committee, update policies and procedures, conduct pharmacy service enhancement activities.)Qualifications Education:
Graduate of an accredited Doctor of Pharmacy (Pharm D.)Licensure/Certification:
California Board of Pharmacy licensure as an intern upon hire, then as a pharmacist within 90 days from hire date.Experience:
None required.Skills:
Excellent verbal and written communication skills. Must demonstrate competency in pharmaceutical calculations. Ability to effectively use the electronic health record and other computer-based programs and software. Ability to apply professional knowledge and judgment to the provision of pharmaceutical care for patients.Standards of Performance:
Demonstrate performance by adhering to established policies and procedures, accreditation requirement established by ASHP and exhibiting the defined characteristics associated with attendance and punctuality.Physical Effort:
