Role:

Join Skilled Nursing Pharmacy as a PM Pharmacist and play a vital role in delivering exceptional pharmaceutical care. We are looking for a dedicated professional to ensure our patients receive the highest quality of medication management and support.

Responsibilities:

Oversee medication dispensing and ensure accuracy in prescriptions. Provide clinical consultations to healthcare staff and patients. Monitor patient medication therapy and assess for potential interactions. Collaborate with interdisciplinary teams to optimize patient outcomes. Conduct medication reviews and participate in quality assurance initiatives. Educate patients and caregivers on medication usage and safety. Requirements:

PharmD or equivalent degree from an accredited pharmacy school. Current California pharmacist license in good standing. Minimum of 2 years of experience in a pharmacy setting, preferably in skilled nursing. Strong clinical knowledge and excellent communication skills. Ability to work independently and collaboratively in a team environment. Detail-oriented with strong problem-solving abilities. Proficiency in pharmacy management software and electronic health records. Commitment to providing compassionate patient care.
