Job Description
Category Pharmacy - Pharmacist Job Type Contract A pharmacy role focused on Transitions of Care and Ambulatory Care is available at a large 400-bed hospital in the San Fernando Valley region of Los Angeles. This position offers an opportunity to impact patient care by working closely with individuals prior to discharge, ensuring they understand their medications and have necessary refills in place to support adherence and positive health outcomes.
Responsibilities:
Provide medication education and counseling to patients before discharge Assist patients in obtaining medication refills prior to leaving the hospital ("meds to beds" program) Support patient adherence to prescribed medication regimens to reduce hospital readmissions Collaborate with healthcare teams within a large county health system encompassing outpatient clinics and hospitalsDesired Experience & Qualifications:
Licensed Pharmacist with current credentials Preference given to new PGY1 residency graduates, though non-PGY1 candidates will be considered Strong communication skills and patient counseling ability Experience or interest in transitions of care or ambulatory pharmacy services is beneficialLocation:
Sylmar, California, at a major county hospital serving diverse communities This is a long-term contract opportunity within a respected county health department hospital system. Pharmacists hired through the agency work on the agency payroll but have the option to apply directly to the County for permanent employment.
