Responsibilities:

Dispense prescription medications accurately according to physician orders and pharmacy policies Provide comprehensive medication therapy management, including IV medication preparation and administration oversight Counsel patients on their prescriptions and infusion therapies, ensuring understanding of dosage, administration, and potential side effects Collaborate closely with healthcare providers and home infusion teams to coordinate care and optimize treatment plans Maintain compliance with all state and federal pharmacy regulations and safety standards Monitor inventory levels and ensure appropriate storage conditions for all pharmaceuticals

Qualifications and Desired Experience:

Licensed Pharmacist with current state pharmacy license IV certification with experience in preparing and managing intravenous medications Previous home infusion pharmacy or related ambulatory care experience preferred Strong knowledge of pharmaceutical standards and regulatory requirements Excellent communication skills for patient counseling and interdisciplinary collaboration Ability to work independently with minimal supervision and maintain high attention to detail
