Category Pharmacy - Clinical Pharmacist Job Type Per Diem A healthcare facility in Manteca, CA is seeking a dedicated Clinical Pharmacist to provide expert infusion pharmacy support on a part-time, per diem basis—working 2 to 3 days per week. This position offers an excellent opportunity for pharmacists with strong IV/infusion and clinical experience, especially those with prior home infusion pharmacy background.
Location:
Manteca, CA (Ideal candidates will reside near Modesto, Stockton, or Merced)
Schedule:
Monday to Friday 8:30 AM - 5:00 PM 2-3 days per week
Responsibilities:
Evaluate physician/prescriber orders for appropriate dosage, drug selection, administration route, and potential interactions. Verify patient information, including height, weight, medical history, lab results, and current medications (prescription and OTC). Maintain compliance documentation adhering to state, federal, and accrediting body requirements such as JCAHO and ACHC. Order and review clinical laboratory tests, collaborating with nurses and physicians to optimize therapy. Provide clinical and technical support to healthcare staff and patients. Implement pharmacy policies and assist in staff training and supervision, including pharmacy technicians and interns. Oversee sterile compounding and quality assurance programs involving validation, environmental, and end product testing. Document all communications with healthcare providers and caregivers in the patient's medical record. Promote patient safety and provide drug therapy education to patients and caregivers.
Qualifications:
Active California pharmacist license in good standing. Proven expertise in IV/infusion and clinical pharmacy practice, with preference for home infusion experience. Familiarity with CareTend pharmacy management system is a plus. This is an ongoing opportunity that requires immediate availability.
