DESCRIPTION UC

San Diego Health is seeking an outstanding clinical pharmacist to join our Transitions of Care team, offering a unique opportunity to integrate advanced clinical practice with teaching and research at the Skaggs School of Pharmacy and Pharmaceutical Sciences. Under general supervision, the Pharmacist II is responsible for the provision and documentation of medications and medication-related services in accordance with standards established by the Department of Pharmacy. This role supports UC San Diego Health's clinical, academic, and research missions through direct patient care, education, and interdisciplinary collaboration. The Pharmacist II will provide direct patient care through a transitions-of-care model and the Heart Failure Clinic, serving patients with advanced heart failure and other complex medical conditions across the continuum of care, from critically ill to stable internal medicine patients. Responsibilities include medication reconciliation, discharge counseling, post-discharge follow-up, and collaboration with interdisciplinary providers to ensure safe, effective, and seamless transitions. Working under Collaborative Practice Agreements, the pharmacist will optimize guideline-directed medical therapy, provide comprehensive patient education and counseling, address adherence barriers, and promote therapeutic outcomes. The position also includes serving as a preceptor for pharmacy residents and students, participating in Medical Center and Departmental committees, and contributing to institution-wide medication-use policy and quality initiatives. The target patient population may include (but is not limited) heart failure patients, general cardiology patients, patients living with HIV, and internal medicine patients.

KEY RESPONSIBILITIES

Provide comprehensive transitions of care services, including admission and discharge medication reconciliation Counsel patients and caregivers on medication therapy at discharge Conduct post discharge medication follow ups and clinical interventions Collaborate with providers and interdisciplinary teams to resolve medication related problems Practice under Collaborative Practice Agreements as applicable Assist with prior authorizations and medication access issues Provide clinical documentation in the electronic health record Serve as a preceptor for pharmacy residents and students Participate in departmental, medical center, and quality improvement initiatives Float across multiple units and locations to meet operational and clinical needs Ensure compliance with state and federal pharmacy regulations

MINIMUM QUALIFICATIONS

Doctorate of Pharmacy degree or equivalent clinical experience. Registered Pharmacist - issued by the state of California (RPH). Incumbents can meet the minimum experience qualifications for this position in 1 of 3 ways: 1) possesses a PGY 2 or 2) possesses a PGY 1 plus 1 year of pharmacist experience, or 3) possesses 4 years of pharmacist experience.

PREFERRED QUALIFICATIONS PGY2

residency or equivalent clinical experience (cardiology or transitions of care (TOC) preferred). Four (4) or more years of pharmacist experience in either cardiology, heart failure, TOC. Experience in transitions of care or discharge focused pharmacy services. Experience working with diverse and complex patient populations. Strong communication, organizational, and teaching skills.

Pay Transparency Act Annual Full Pay Range:

$190,488 - $227,592 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent:

$91.23 - $109.00 Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
