Job Description:

We are a growing dermatology practice seeking a Clinical Pharmacist to provide direct patient care. Pharmaceutical care services include but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events.

Responsibilities:

Collaborate with healthcare professionals to develop and implement patient care plans Monitor patient medication therapy for effectiveness, safety, and adherence Provide drug information and education to healthcare providers and patients Maintain accurate patient medication records and documentation Ensure compliance with all relevant laws, regulations, and standards

Requirements:

Pharm D. degree and current CA Pharmacy license (required). ASHP-accredited residency (preferred). 2 years inpatient pharmacy experience (preferred). Exceptional communication, interpersonal, organizational and follow-through skills. Knowledge of and ability to perform age specific and disease specific pharmacokinetic calculations.
