Key Responsibilities:
Dispense and verify medications with a focus on home infusion therapy Provide comprehensive pharmaceutical care to patients receiving IV medications Collaborate with healthcare teams to ensure safe and effective medication use Maintain accurate records and comply with regulatory standards Educate patients and caregivers regarding medication administration and safety
Qualifications and Desired Experience:
Licensed Pharmacist with valid state license Certification in IV admixture and administration Experience in home infusion pharmacy services Strong knowledge of pharmacy operations and protocols Excellent communication and patient care skills
