Job Overview We are seeking a dedicated and detail-oriented Pharmacist to join our healthcare team. Responsibilities Dispense medications accurately using pharmacy software systems while adhering to all legal and safety standards Conduct thorough patient assessments to determine appropriate medication therapy, including pediatrics and complex cases Collaborate with healthcare providers to develop and modify medical management plans, ensuring optimal pain management and immunization protocols Prepare compounded medications and perform aseptic techniques for IV infusions, including IV insertion and infusion therapy Manage inventory levels efficiently, ensuring the availability of medications while minimizing waste and expiration risks Maintain detailed documentation within Electronic Health Record (EHR) systems, ensuring compliance with HIPAA regulations Provide patient education on medication administration, potential side effects, and medication safety practices Support pharmacy technician staff by overseeing medication preparation processes and verifying their work for accuracy Participate in patient care activities such as medication reconciliation, patient service initiatives, and clinical pharmacy interventions in hospital or acute care settings Skills Extensive knowledge of pharmacy operations within LTC pharmacy, critical care, and hospital environments Proficiency with pharmacy software applications and EMR/EHR systems for accurate record-keeping Strong understanding of medical terminology, physiology, anatomy, and pharmacology principles relevant to diverse patient populations Experience with compounding medications using aseptic techniques and managing IV infusions safely Skilled in immunizations administration and medication management strategies for pediatrics and adult patients Ability to perform patient assessment effectively to guide clinical decision-making in acute care settings Familiarity with inventory management practices to ensure medication availability while controlling costs Knowledge of HIPAA regulations to protect patient confidentiality at all times Prior experience working with pharmacy technicians to ensure smooth workflow processes Excellent communication skills for engaging with patients, healthcare providers, and team members effectively Join us in delivering exceptional pharmaceutical care that enhances patient health outcomes!
