At Lee Health , pharmacists are clinical partners practicing at the top of their license. In this advanced role, you'll work alongside a multidisciplinary heart failure team to optimize therapy for patients with complex cardiac conditions. Practice in a Brand-New Advanced Heart Failure Center Join the new Advanced Heart Failure Center at the Lee Health Heart Institute , bringing next-generation cardiac care to Southwest Florida. You'll help manage patients requiring: Inotrope infusion therapy LVAD and heart transplant care Complex conditions including cardiac amyloidosis, hypertrophic cardiomyopathy, and cardiac sarcoidosis This program includes: A dedicated 27-bed Advanced Heart Failure Unit Specialized ambulatory clinic Remote cardiac device monitoring Multidisciplinary transitions-of-care model You'll collaborate with heart failure specialists, cardiology providers, nurses, and expanding support services in a highly integrated care environment.
What You'll Do:
Collaborative Practice Agreement (CPA)based medication management , including initiation, titration, and monitoring of therapy Optimization of heart failure guideline-directed medical therapy (GDMT) Medication reconciliation and transitions-of-care support to reduce readmissions Patient education and counseling on medications, adherence, and side-effect management Monitoring labs, vital signs, and clinical response to therapy Active participation in clinic visits, care coordination, and EMR documentation Comprehensive Medication Therapy Management (MTM) Direct collaboration with specialty pharmacy services to ensure continuity of high-cost therapies Support with prior authorizations and medication access , helping remove barriers to care Professional Development Obtain Consultant Pharmacist license within 36 months Earn HFSA Heart Failure Certification (HF-Cert) within 3 years Why Lee Health Team-based, collaborative culture Cardiovascular program known for innovation and excellence High-impact work with complex patients Strong support for specialization and growth
Requirements Education:
PharmD required or BS Pharmacy degree with equivalent and acceptable experience from a College of Pharmacy accredited by American Association of Colleges of Pharmacy (AACP).
Experience:
Five years of professionally responsible pharmacist experience required for Bachelor's degree holders. One year of recent inpatient hospital experience as staff pharmacist or pharmacy intern in a general medical/surgical hospital preferred for PharmD candidates.
Certification:
BLS (American Heart Association/Basic Life Support) required (must be obtained within first 90 days of hire). ACLS (AHA) and/or PALS (AHA), depending on practice site.
License:
State of Florida Department of Professional Regulation, Registered Pharmacist Licensure.
Other:
Strong communication and interpersonal skills, data entry skills, analytical and basic math skills required.
