In this vital role, you will provide comprehensive pharmaceutical care within a fast-paced hospital environment, ensuring optimal medication therapy management for diverse patient populations. Your knowledge of physiology, anatomy, and medical terminology will be essential in delivering precise patient assessments and tailored medication plans. We are seeking a motivated professional with a passion for patient service and a commitment to excellence in clinical pharmacy practice. Duties This role entails reviewing patient lab PGX reports and making recommendations for changes in the meds based on the labs Requirements Valid pharmacist license with current registration in the relevant jurisdiction Prior hospital experience, including ICU or emergency room settings, is highly preferred Demonstrated experience in managed care environments and hospital medicine practices Strong understanding of medical terminology, anatomy, physiology, and acute care principles Experience with pharmacy residency programs or post-residency clinical pharmacy practice is desirable Proven skills in patient service, patient care, and medication management within a hospital setting Familiarity with aseptic techniques for IV infusion preparation and handling hazardous medications Critical care experience including trauma center operations and pediatric pharmacy (Pediatrics) is advantageous Ability to perform detailed patient assessments and contribute to multidisciplinary team efforts in pain management and acute pain control Join us to make a meaningful difference through expert pharmaceutical care!
