VBC Clinical Pharmacist

PRIMARY FUNCTION

The Clinical Pharmacist reports to the Enterprise Innovation Pharmacist and provides direct patient-centered medication management services across the Pediatric Associates enterprise. This role focuses on optimizing medication therapy, improving adherence, and supporting value-based care initiatives.

ESSENTIAL DUTIES

Patient Care

• Conduct comprehensive medication reviews and reconciliations.
• Identify and resolve medication-related problems (non-adherence, interactions, duplications, gaps in therapy).
• Provide patient counseling via phone or telehealth.
• Support transitions of care and chronic disease management.
• Develop care plans in collaboration with providers. Quality & Value-Based Care
• Support Medicare Stars and Medicaid pharmacy quality measures (e.g., PDC).
• Conduct targeted outreach to close medication-related gaps.
• Document interventions per regulatory and payor requirements.
• Assist with formulary alignment and biosimilar transitions. Collaboration
• Communicate therapeutic recommendations to providers.
• Participate in interdisciplinary care coordination.
• Escalate complex cases as needed.

SUPERVISORY RESPONSIBILITIES

• No supervisory responsibilities

QUALIFICATIONS

• Doctorate of Pharmacy required
• Active pharmacist license in good standing
• 1+ years clinical pharmacy experience in ambulatory care, pediatrics, or managed care preferred but not required SKILLS
• Strong patient counseling and communication skills.
• Knowledge of chronic disease guidelines.
• Ability to interpret pharmacy data and document interventions.
• Proficient in EMR and Microsoft Office.

WORKING CONDITIONS

• Patient-facing (telehealth).
• May be remote (U.S.-based, HIPAA-compliant workspace required).
• Primarily computer-based with deadlines and outreach targets.
