Job Description
PRIMARY FUNCTION
The Clinical Pharmacist reports to the Enterprise Innovation Pharmacist and provides direct patient-centered medication management services across the Pediatric Associates enterprise. This role focuses on optimizing medication therapy, improving adherence, and supporting value-based care initiatives.ESSENTIAL DUTIES
Patient Care- Conduct comprehensive medication reviews and reconciliations.
- Identify and resolve medication-related problems (non-adherence, interactions, duplications, gaps in therapy).
- Provide patient counseling via phone or telehealth.
- Support transitions of care and chronic disease management.
- Develop care plans in collaboration with providers. Quality & Value-Based Care
- Support Medicare Stars and Medicaid pharmacy quality measures (e.g., PDC).
- Conduct targeted outreach to close medication-related gaps.
- Document interventions per regulatory and payor requirements.
- Assist with formulary alignment and biosimilar transitions. Collaboration
- Communicate therapeutic recommendations to providers.
- Participate in interdisciplinary care coordination.
- Escalate complex cases as needed.
SUPERVISORY RESPONSIBILITIES
- No supervisory responsibilities
QUALIFICATIONS
- Doctorate of Pharmacy required
- Active pharmacist license in good standing
- 1+ years clinical pharmacy experience in ambulatory care, pediatrics, or managed care preferred but not required SKILLS
- Strong patient counseling and communication skills.
- Knowledge of chronic disease guidelines.
- Ability to interpret pharmacy data and document interventions.
- Proficient in EMR and Microsoft Office.
WORKING CONDITIONS
- Patient-facing (telehealth).
- May be remote (U.S.-based, HIPAA-compliant workspace required).
- Primarily computer-based with deadlines and outreach targets.
