Job Description
Overview The Hospital Pharmacist provides services including but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Works directly with physicians, other health professionals and patients to provide all aspects of patient care, including clinical and distributive tasks, for patients served within our health system. Helps patients and healthcare professionals achieve clinical outcomes and monitor progress toward common goals. Supervises other pharmacy staff in the absence of formal departmental leadership. BS Pharmacy with equivalent clinical experience Doctor of Pharmacy (PharmD) preferred Completion of an ASHP accredited PGY-1 pharmacy residency desirableExperience:
Proficiency in navigating the electronic medical record One or more years of hospital pharmacy experience preferred License(s)/Certification(s): Iowa Pharmacist license in good standing or eligible for licensure Must attain a license to practice Pharmacy in Iowa within six months of employment Must be directly supervised by a licensed pharmacist until Iowa Pharmacist license is obtained Must maintain current license to practice pharmacy in Iowa and be in good standing with the Iowa Board of Pharmacy Examiners Valid driver's license when driving any vehicle for work-related reasonsSimilar jobs in Des Moines, IA
