Job Description

Overview:
Unity Point St. Luke's-Downtown Full-time, Nights 9pm-7:30am, 7 on,7 off Provides services including but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Works directly with physicians, other health professionals and patients to provide all aspects of patient care, including clinical and distributive tasks, for patients served within our health system. Helps patients and healthcare professionals achieve clinical outcomes and monitor progress toward common goals. Supervises other pharmacy staff in the absence of formal departmental leadership. Responsibilities:
Verifies all orders generated on shift, keeps work flow up to date and current, and cleans up old orders as soon as possible. Completes verbal hand off communication to next shift, to ensure continuity of quality patient care and follow through in interventions. Screens and affirms appropriateness of orders including allergy check, formulary compliance, pregnancy & lactation status, dosing, duplicate orders etc. Documents adverse drugs reactions & medication errors in the RL system as they occur throughout the medical center. Fields and investigates drug information questions. Uses resources appropriately and colleagues as needed to ensure timely and accurate responses. Documents as necessary. Checks and assures high quality of IV Room products every time. Ensures timeliness of delivery of final product, utilizing means of maximum efficiency. Enters TPN orders into compounder computer system accurately and timely.
Qualifications:
Education:
Graduate of accredited College of Pharmacy required. Pharm.

D. Residency preferred.
Experience:
1-3 years of job related experience required. One year of hospital clinical experience, Residency preferred. License(s)/Certification(s): Current licensure in good standing to practice as a Registered Pharmacist in Iowa or licensure eligible in Iowa required. Valid driver's license when driving any vehicle for work-related reasons required. Board Certified Oncology Pharmacist (BCOP) or Board-Certified preferred. Pharmacotherapy Specialist (BCPS) or Board-Certified Ambulatory Care Pharmacist (BCAPS) preferred.
