St. Luke's is seeking an Ambulatory Care Pharmacist to join our team. This position is embedded within several primary care clinics in the Wood River area of Idaho (Hailey and Ketchum). This role is an essential part of the health care team within our clinics. The Wood River Valley offers a unique opportunity to practice ambulatory care pharmacy in a close-knit community while enjoying year-round outdoor recreation and a high quality of life. Pharmacists in this role independently manage chronic disease states while working closely with primary care providers. The St. Luke's Ambulatory Pharmacy Department includes pharmacists embedded in clinics across the health system within the following specialties: internal medicine, family medicine, endocrinology, anticoagulation, heart failure, lipidology, cystic fibrosis, and pediatric specialties. What you can expect from this role- An independent clinic-based pharmacist practice with broad scope of practice
- Management of a fast-paced clinic schedule of 8-12 patient appointments per day
- Delivering comprehensive support to providers and patients by answering drug information inquiries, resolving pharmacy-related issues, and providing medication education
- Working as part of a multidisciplinary team with physicians, advanced practice practitioners, behavioral health specialists, and care managers
- Collaboration with clinical pharmacy technicians
- Opportunities for precepting pharmacy students and residents
Hours:
Part Time; Monday - Friday (either 2.5 days per week or rotating 2 or 3 days per week every 2 weeks): 8 am to 5 pm- Please note: there is an opportunity to work an additional 0.5 FTE with the inpatient pharmacy team. Please inquire for details. Preferred Qualifications for this Role
- Previous experience with the clinical management of primary care based chronic diseases
- Completion of a PGY2 Ambulatory Care Residency, completion of a PGY1 Pharmacy Residency with strong ambulatory care focus, or equivalent ambulatory care experience
- Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP) What's In It For You At St.
