Position Overview:
We are seeking a motivated and dependable Pharmacy Extern to join our Meds2Beds Program on a weekend-only basis. This role supports timely medication delivery and patient education for those being discharged from the hospital, playing a key role in reducing readmissions and improving patient outcomes.
Key Responsibilities:
Deliver prescribed medications directly to discharged patients. Provide patient counseling on medication usage, potential side effects, and the importance of adherence. Collaborate with pharmacists, nurses, and care teams to ensure smooth patient transitions. Accurately document prescriptions and patient interactions in compliance with hospital and pharmacy protocols. Provide coverage during weekends and holidays as needed.
Qualifications:
Enrolled in an accredited Pharmacy program. Holds a valid pharmacy intern/extern license. Current Basic Life Support (BLS) certification. Strong interpersonal and communication skills. Detail-oriented and committed to patient-centered care.
Schedule :
Weekends Only (Saturday and Sunday) [Insert shift times, e.g., 8 AM - 4 PM, or specify "varied weekend shifts as assigned"] Occasional holiday coverage required Why Join Us?
