St. Luke's is seeking an Ambulatory Care Pharmacist to join our team. This position is embedded within a large primary care clinic in Twin Falls, Idaho. This role is an essential part of the health care team within our clinics. Pharmacists in this role independently manage chronic disease states while working closely with primary care providers. The St. Luke's Ambulatory Pharmacy Department includes pharmacists embedded in clinics across the health system within the following specialties: internal medicine, family medicine, endocrinology, anticoagulation, heart failure, lipidology, cystic fibrosis, and pediatric specialties. What you can expect from this role
- An independent clinic-based pharmacist practice with broad scope of practice
- Management of a fast-paced clinic schedule of 8-12 patient appointments per day
- Delivering comprehensive support to providers and patients by answering drug information inquiries, resolving pharmacy-related issues, and providing medication education
- Working as part of a multidisciplinary team with physicians, advanced practice practitioners, behavioral health specialists, and care managers
- Collaboration with clinical pharmacy technicians
- Opportunities for precepting pharmacy students and residents
Hours:
Monday -Friday:
8 am to 5 pm Preferred Qualifications for this Role- Previous experience with the clinical management of primary care based chronic diseases
- Completion of a PGY2 Ambulatory Care Residency, completion of a PGY1 Pharmacy Residency with strong ambulatory care focus, or equivalent ambulatory care experience
- Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP) At St.
