Full-time Description:
Responsible for providing pharmaceutical care services to patients including appropriateness, safety and efficacy of therapy. Works as part of a multi-disciplinary team including providers, nurses, and other staff. Assists in day to day pharmacy operation activities as needed.

Yes Primary Functions:
1. Counsel, advise and educate patients on medication management and the relationship to diseases; reviews medication regimens for appropriateness, accuracy, safety, and completeness; evaluates any adverse side-effects of medicines or potential interactions with other medicines/treatments; and maintains documentation of patient communication. 2. Educate, communicate and coordinate with physicians, dentists, nurses, VNA management and other staff regarding orders and issues related to patient care and pharmacy related topics. 3. Evaluate equipment, supplies, medication, and educational materials being used for pharmacy effectiveness as well as cost effectiveness. 4. Represent the organization before professional and community groups to promote VNA services and participate in networking activities. 5. Precept pharmacy students, whose academic activities include practical and theoretical knowledge, along with direct experience. 6. Perform the following in the pharmacy: Dispense pharmaceuticals and related supplies in accordance with all federal, state and local laws and the standard of practice that governs the profession of pharmacy. 7. Ensure the ordering and safe storage of pharmaceuticals and, in some cases, arranging the delivery of prescription medicines to patients. 8. Responsible for the oversight of pharmacy technicians during service hours. Familiarizes and complies with all VNA policies and procedures. Meets standards of behavior expectations. Follows established guidelines for use and/or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy Officer immediately. Failure to comply with HIPAA policies and procedures will result in disciplinary action, up to and including termination of employment. Other duties as assigned.
Qualifications:
Graduate of an ACPE accredited College of Pharmacy. PharmD degree preferred. Maintains current CPR Certification. 3. Two years of clinical pharmacy experience preferred. 4. One year of professional experience in the compounding, dispensing, and preservation of pharmaceuticals. 5. Licensed in state of Illinois to practice pharmacy without restrictions required. 6. Health policies and requirements must be met. 7. Computer Literate.
Essential Functions/Physical Requirements:
