Clinical Pharmacist Terre Haute, IN Position Details Full-Time, Permanent Position Day Shift | Varied Schedule Pharmacy & Therapeutics Department Up to $20,000
Sign-On Bonus Compensation Hourly Pay:
$53.37 - $80.06 per hour
Annual Salary:
Approximately $111,010 - $166,525 annually Comprehensive Benefits Package Position Summary We are seeking a Clinical Pharmacist to join a collaborative healthcare team dedicated to optimizing medication therapy and improving patient outcomes. This role plays an integral part in managing medication use throughout the hospital, including medication selection, prescribing support, procurement, administration review, and patient education. The Clinical Pharmacist works closely with physicians, nurses, and interdisciplinary teams to ensure safe, effective, and evidence-based medication management while supporting quality improvement initiatives and clinical pharmacy services. Key Responsibilities Provide drug information and medication consultation services to patients, providers, nursing staff, and ancillary departments Perform pharmacokinetic and drug dosing consultations while supporting medication management initiatives Participate in interdisciplinary committees including Medication Safety, Infection Control, Antimicrobial Stewardship, Sepsis, Stroke, Pain Management, and Quality Improvement teams Support formulary management, medication-use evaluations, and pharmacy performance improvement initiatives Develop and deliver educational programs for healthcare providers, pharmacy staff, and patients Collaborate with providers through Collaborative Drug Therapy Management (CDTM) protocols to optimize chronic disease management Provide direct patient care services, including anticoagulation management, point-of-care INR testing, medication monitoring, and patient counseling Maintain compliance with pharmacy regulations, accreditation standards, and organizational policies Participate in special projects, clinical documentation activities, and departmental initiatives Education & Experience Requirements Doctor of Pharmacy (PharmD) degree required Licensed or eligible for Pharmacist licensure in the State of Indiana Must obtain Indiana pharmacist licensure within 75 days of employment One to two years of ASHP-accredited residency training or equivalent clinical experience preferred Ambulatory care experience preferred Board Certification preferred upon first eligibility Preferred Qualifications Strong clinical knowledge across acute care and ambulatory pharmacy practice Experience with anticoagulation management and chronic disease state management Excellent communication and interdisciplinary collaboration skills Passion for patient education and evidence-based practice Commitment to professional development and continuous learning Benefits Up to $20,000 Sign-On Bonus Comprehensive Benefits Package Paid Time Off Starting Day One Tuition Reimbursement Up to $5,250 Annually 403(b) Retirement Plan with Employer Match Career Growth and Advancement Opportunities Employee Success Sharing Program Supportive and Collaborative Clinical Environment This is an excellent opportunity for a Clinical Pharmacist seeking a patient-focused role with strong clinical involvement, interdisciplinary collaboration, and opportunities for professional growth.
