Job Description
JOB SUMMARY
The Specialty Operations Pharmacist is responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication-use systems as well as deliver optimal medication therapy to patients with a broad range of disease states. Specialty Operations Pharmacists proficiently provide direct patient-centered care and integrated pharmacy operational services in a decentralized practice setting with physicians, nurses, and other hospital personnel. These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management within specialty patient care services and to ensure pharmaceutical care programs are appropriately integrated throughout the institution.DUTIES AND ESSENTIAL FUNCTIONS
Participates in the dispensing function of specialty medications including patient communication, cold chain distribution, appropriate labeling, oversight of medication purchases, fulfillment of REMS responsibility, packaging, and mail order services Overall performance of below indicated essential job duties: Oversees Access to Care Services including Benefits Investigation, Prior Authorization requirements, HUB support, copay assistance, foundation support, and patient assistance services during the on-boarding process for specialty medications Functions as liaison between provider and health insurer for optimizing medication therapy. Advocates to health insurer on patients' behalf for access to required medications. Participates in Peer-to-Peer reviews as allowed Create and maintain clinical pharmacy pathways for initiation and monitoring of specialty medication therapy for self-administered medications, including step therapy requirements, patient education, and monitoring of applicable laboratory, pharmacokinetic, and pharmacogenomic results Facilitates experiential education and practice advancement Maintains proficiency in preceptor roles and actively emulates education/mentorship skills Develops student, resident, and pharmacy/staff training experiences/competencies and creates new relationships for teaching and training opportunities Identifies, designs, and implements improvements in the medication-use system to advance patient safety, maximize therapeutic outcomes, or control costsEDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS
Minimum requirement: Graduate of an ACPE school of pharmacy with a BS Pharmacy or PharmD degree Knowledge ofTJC, USP
, , and , and federal and state regulations required Kentucky Board of Pharmacy Pharmacist license Ohio Pharmacist license obtained within 90 days of hire Achieves and maintains American Heart Association Basic Life Support (BLS) Achieves and maintains American Heart Association Advanced Cardiac Life Support within first 90 days of employment Preferred qualifications: Completion of PGY-1 or PGY-2 Specialty Pharmacy Residency from an ASHP-accredited programWORKING ENVIRONMENT

Occasionally required to maintain in a stationary position (standing or sitting). Frequently required to walk and move about. Frequently required to operate a computer and other equipment. Frequently required to use fine motor skills to handle and operate instruments/devices. Constantly required to lift and/or move items up to 10 pounds. Frequently required to lift and/or move items up to 50 pounds. Occasionally required to lift and/or move items up to 100 pounds. Constantly required to communicate with patients and other care team requiring talking and hearing. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
