How you'll contribute A Clinical Staff Pharmacist who excels in this role: Evaluate patient drug profiles to optimize therapy and ensure safety. Collaborate with physicians and healthcare providers to improve medication therapy outcomes. Participate in medication reconciliation to ensure accurate patient medication histories. Provide pharmacokinetic assessments and therapeutic drug monitoring. Attend interdisciplinary team rounds and meetings to support collaborative care planning. Education:
Bachelor's Degree Required;
Doctoral Degree Preferred Licenses:
