Job Description

POSITION DESCRIPTION
The MTM Pharmacist is responsible for providing comprehensive medication management services to optimize patient outcomes, improve medication adherence, reduce medication-related risks, and support organizational quality and value-based care initiatives through direct patient care and provider collaboration his position requires compliance with CommuniHealth Services' written standards, including the Compliance Plan and policies and procedures (including the Code of Conduct), as well as assistance in the continuous monitoring, reporting and corrective action processes of corporate compliance.

Conduct comprehensive medication reviews (CMRs) to identify, resolve, and prevent medication-related problems. 2. Perform targeted medication reviews (TMRs) and interventions to optimize therapeutic outcomes and medication safety. 3. Evaluate medication regimens for appropriateness, effectiveness, safety, adherence, and cost-effectiveness. 4. Develop and implement individualized medication action plans in collaboration with patients and healthcare providers. 5. Provide patient-centered medication counseling and education to improve understanding, adherence, and self-management. 6. Complete medication reconciliation during care transitions, including hospital discharge and provider referrals. 7. Collaborate with clinical pharmacists and/or other designated personnel to optimize medication therapy. 8. Identify and address gaps in care, preventive health opportunities, and evidence-based treatment recommendations. 9. Support chronic disease management initiatives, including diabetes, hypertension, hyperlipidemia, asthma, and other disease states. 10. Monitor and improve medication adherence through patient outreach, intervention programs, and follow-up activities. 11. Document clinical interventions, recommendations, patient encounters, and outcomes within the electronic health record and other required systems. 12. Assist in achieving organizational quality, population health, and value-based care goals through medication optimization strategies. 13. Analyze patient medication profiles to identify therapeutic duplications, drug interactions, adverse effects, and opportunities for therapy optimization. 14. Participate in interdisciplinary care team meetings and provide clinical medication expertise. 15. Assist with Annual Wellness Visits, Transitional Care Management services, and other clinical pharmacy initiatives as assigned. 16. Develops, documents and tracks MTM metrics through the MTM dashboard. 17. Utilizes MTM metrics to 18. Assists as a backup staff pharmacist for the retail services pharmacy department as needed and must adhere to the staff pharmacist's job duties and responsibilities. 19. Maintains up-to-date knowledge of the profession by participating in organization memberships; by attending meetings, lectures, in-services, and seminars; by reading journals; and by complying with Louisiana Board of Continuing Education 20. Reviews and interprets physicians' orders for appropriateness relevant to other departments (e.g., drugs for diagnostic procedures, laboratory tests) to ensure that overdoses or under-doses have not been prescribed and that interactions, incompatibilities or toxic compounds will not result from the combination of medications 21. Performs various clerical, billing, and technical duties necessary to support the operation of the pharmacy 22. Provides counseling services to patients regarding their medication use 23. In addition to the outlined duties above, as a patient-centered medical home, the incumbent fulfills the principles and theories outlined in organizational policies including assisting navigating patients as needed through the health system, making appropriate referrals to case/care management, communicating with the healthcare team to address patient/family/caregiver specific needs, etc. 24. Completes required CHS training per policy, maintains compliance with and knowledge of all CHS departmental, facility specific and system-wide policy and procedure, attends mandatory departmental meetings and/or training, and follows CHS Mission, Vision and Values in delivery of all services 25. Participates in all appropriate meetings within the organization. 26. Maintains a working knowledge of all health information management issues such as HIPAA, Corporate Compliance, and other such healthcare regulations as well as compliance with HRSA regulations 27. Education/licensure:
1. Graduate of a Bachelor of Science in Pharmacy or PharmD (required) 2. State of Louisiana Pharmacy License (required) 3. CPR certification (required)
Skills/Experience:
1. Must be able to read, write and speak the English language. 2. Knowledge of drug therapy and disease state management 3. Knowledge of state and federal regulation related to pharmacy services 4. Knowledge of outcomes 5. Knowledge of clinical evidence-based medicine guidelines for chronic disease state management 6. Must possess excellent communication (verbal and written) skills 7. Must possess problem-solving skills 8. Must be able to work independently and be a self-starter. 9. Must possess the ability to adapt to changing patient and departmental needs. 10. Must be able to use Microsoft, EHR software, and all pharmacy software proficiently
Environmental/Working Conditions:
1. Works in well-lighted/ventilated office area of the facility. 2. Is subject to frequent interruptions. 3. Is involved with personnel, patients, visitors, government agencies/personnel, etc., under all conditions/circumstances. 4. Is subject to hostile and emotionally upset personnel, patients, visitors, etc. 5. Is willing to work beyond normal working hours: weekends, holidays, and other shifts/positions, as necessary. 6. Attends and participates in continuing educational programs designed to keep you abreast of changes in your profession. 7. Is subject to cuts, bruises, falls, burns from equipment, odors, etc., throughout the workday. 8. May be exposed to blood borne pathogens, infectious waste, diseases, conditions, etc., including the potential exposure to the AIDS and Hepatitis B viruses.
Physical Demands:
1. Must be able to handle occasional lifting and pulling. 2. Must be able to move intermittently throughout the workday. 3. Must be able to function independently, have personal integrity, and the ability to work effectively with patients, personnel, and support agencies. 4. Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
