Job Description

Ready to practice at the top of your license and actually see the impact of your work? Bolster Rx is launching a brand‑new patient‑first specialty pharmacy program at Lane Regional Medical Center and we're hiring multiple Clinical Specialty Pharmacists! If you're passionate about clinical counseling, medication access, and improving outcomes , this role puts your expertise exactly where it matters most. You'll help design clinical workflows, shape patient care models, and establish a specialty service line that will transform care for our community.
What You'll Do:
Provide direct patient care through medication therapy evaluations, consultations, chart review, and ongoing therapy assessments Collaborate with prescribers to optimize treatment plans using evidence-based guidelines and patient-specific needs Educate patients on medication use, side effects, adherence strategies, and lifestyle modifications Ensure safe, coordinated care by performing medication reconciliation, monitoring adherence, and identifying interactions or adverse reactions Support medication access by managing prior authorizations, appeals, financial assistance, and care coordination
What You Bring:
A current, unrestricted Louisiana pharmacist license Strong knowledge of pharmacotherapy and evidence‑based medicine Excellent communication and patient counseling skills Ability to work independently and collaboratively within an interdisciplinary team Proficiency with EHR systems and strong documentation habits Dedication to supporting your community and a passion for helping others!
