Job Description

Reporting to the Director of Pharmacy, the Clinical Pharmacist provides comprehensive pharmaceutical care across multiple practice settings, including inpatient generalist clinical pharmacy services, ambulatory anticoagulation management, and pain stewardship. The Clinical Pharmacist functions as a generalist within the main pharmacy while also delivering ambulatory care services in the Anticoagulation Clinic and serving as a Pain Stewardship Pharmacist in collaboration with multidisciplinary stakeholders. The Clinical Pharmacist provides pharmaceutical care through the verification, filling, and dispensing of medication orders. Supervises and verifies the accuracy of all work performed by pharmacy technicians and students. Works independently with limited supervision. Develops and promotes collaborative and collegial relationships with medical staff, nursing, and allied health professionals. The Clinical Pharmacist provides comprehensive pharmaceutical care across assigned practice areas through integrated patient assessment and monitoring, standardization of drug distribution, clinical service, teaching/education, and pharmacy representation among the medical and nursing staff. Supports all clinical pharmacy services and initiatives. Additional responsibilities include providing comprehensive ambulatory anticoagulation management appropriate to the age of the patients served by the Anticoagulation Clinic, as directed by the Clinic's Medical Director. Line responsibility to the Anticoagulation Clinic Supervisor.
Minimum Qualifications:
Education:
Graduate of an accredited college/university with a B.S. in Pharmacy or a Pharm.

D. degree is required.
Experience:
A minimum of 2 years of experience in a hospital pharmacy setting is required. Licensure and/or
Certification:
Active licensed Pharmacist in the Commonwealth of Massachusetts (or eligible for reciprocity) is required Board Certification or Completion of a Specialty Certificate in area of practice is desirable
Skills:
Excellent customer service skills. Must be able to read, write, and communicate in English. Computer skills required. Ability to maintain confidential information.
