Job Summary:
We are seeking a highly skilled and dedicated Pharmacist to join our team. As a Pharmacist, you will be responsible for providing exceptional patient care by ensuring the safe and effective use of medications. Your expertise and knowledge in pharmacology will play a vital role in improving patient outcomes and promoting overall wellness.
Responsibilities:
  • Conduct patient assessments to determine appropriate medication therapy
  • Collaborate with healthcare professionals to develop comprehensive treatment plans
  • Provide medication counseling to patients regarding proper usage, potential side effects, and drug interactions
  • Monitor patients' response to medication therapy and make necessary adjustments
  • Ensure accurate and timely dispensing of medications
  • Perform medication reconciliation to prevent adverse drug events
  • Conduct utilization reviews to optimize medication therapy
  • Administer vaccines and review patient's immunization history
  • Collaborate with pharmacy technicians to ensure accurate medication preparation and dispensing
  • Maintain accurate patient records and documentation
Qualifications:
  • Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy
  • Valid pharmacist license in the state of MA
  • Strong knowledge of pharmacology, anatomy, and physiology
  • Experience in patient assessment and medication management
  • Proficiency in utilizing pharmacy software systems for prescription processing and inventory management
  • Excellent communication and interpersonal skills for effective patient counseling and collaboration with healthcare professionals
