We are committed to advancing innovative clinical pharmacy programs that improve patient outcomes and support high-quality healthcare delivery for underserved populations. Position Overview The On-Site Clinical Pharmacist collaborates closely with physicians, nurses, behavioral health teams, and healthcare leadership to deliver comprehensive pharmaceutical care and medication management services across multiple correctional healthcare facilities in the Jessup, Maryland region. Key Responsibilities Deliver direct patient care in ambulatory and institutional healthcare settings Evaluate and optimize medication therapy plans Conduct comprehensive medication reviews and clinical interventions Manage chronic disease states including HIV, Hepatitis C, diabetes, hypertension, asthma/COPD, and pain management Participate in and chair Pharmacy & Therapeutics (P&T) meetings Perform complex drug utilization reviews and formulary management Conduct high-acuity patient case reviews Provide pharmacokinetic monitoring and renal/hepatic dose adjustments Support antibiotic stewardship and infectious disease management initiatives Educate patients and healthcare staff Participate in quality improvement and administrative initiatives
Qualifications Required:
Pharm.

D. degree from an accredited school of pharmacy Active Maryland Pharmacist license Minimum of one-year postgraduate residency training (PGY1) or 3 years post graduate experience
Preferred:
