Job Description Medication Order Verification:
Review and interpret electronic physician orders to confirm accurate drug choices, safe dosages, and proper administration routes .
Clinical Monitoring & Interventions:
Evaluate patient profiles for allergies, drug-drug interactions, and lab results. They frequently perform pharmacokinetic dosing for high-risk medications.
Compounding & Dispensing:
Oversee or personally prepare specialized medications, including large-volume parenterals, intravenous (IV) piggybacks, total parenteral nutrition (TPN), and chemotherapy infusions under strict sterile guidelines (such as USP <797>/<800>).
Interdisciplinary Collaboration:
Participate in medical rounds with physicians and nurses to recommend evidence-based therapies and assist in medical emergencies.
Transitions of Care:
Conduct medication reconciliation during a patient's admission and provide comprehensive drug counseling to patients upon hospital discharge.
Staff Supervision:
Manage, review, and delegate tasks to pharmacy technicians, interns, and students to maintain accurate inventory and cart distributions. Requirements 2 years inpatient pharmacist experience

] Able to work in the Anticoagulation Clinic (4-6 pts a day) and perform Oncology compounding (pharmacist do all the IV compounding)
