Educates and acts as a consultant to patients/family and members of the health care team related to optimal drug therapy and participates in process improvement activities and drug use evaluations. Works with multiple medication automation systems. Primary Responsibilities Provide comprehensive clinical pharmacy services across ICU, progressive care, and emergency department settings, including participation in codes, rapid response events, and acute stroke response. Ensure safe and effective medication use through pharmacokinetic dosing, antimicrobial stewardship collaboration, and medication safety oversight within assigned patient care areas. Collaborate routinely with interdisciplinary team members to support optimal patient outcomes. Serve as a preceptor and mentor for PGY-1 and PGY-2 pharmacy residents, promoting clinical development and professional growth. Participate in residency program activities, including medication use evaluations (MUEs), research initiatives, and acute care skills training for resident onboarding. Contribute to PGY-2 critical care residency programming through precepting, MUEs, and research mentorship. Inpatient pharmacy operational responsibilities included. Interprets and screens medication orders for appropriate agent, dose, route, allergies, and potential drug interactions. Safely compounds and dispenses medications for administration to patients. Evaluates patients, orders appropriate laboratory tests, and interprets serum drug levels utilizing pharmacokinetic principles; provides prescribers with dosage regimen recommendations to optimize therapeutic outcomes, reduce patient risk, and monitor the costs associated with drug therapy. Provides appropriate supervision and direction within appropriate skill level to technicians, residents, interns, and other support personnel. Engages in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities. Completes required continuous training and education, including department and Spectrum Health specific requirements on a timely basis. Provides education and counseling to patients/family/significant others/members of the health care team in order to optimize outcomes and appropriate use of medications. Ensures that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements. Qualifications Required Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy 2 years of relevant experience hospital pharmacy Preferred American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice Preferred LIC-Clinical Pharmacist
